Job Description

Social Worker

Department:               IPA

Job Summary:           Works collaboratively with physicians, office staff and the RN Care Managers to assist patients through the continuum of care settings utilizing community and other resources in an effort to improve patient outcomes and well-being.

Reports to:                 Manager, IPA

 

 

Essential Functions of the Position:

  • Evaluates and identifies patients who need assistance
    • Assesses patients’ needs, situations, strengths and support networks
    • Interfaces with primary care physicians, specialists and RN Care Managers to identify patients in need of social assistance
  • Researches and assists patients with short and long term placement and living arrangements
  • Acts as patient advocate
    • Advocates for and helps patients get resources that would improve their well-being. This includes but is not limited to food stamps, housing, transportation, medications, financial aid
    • Acts as a liaison between the providers and the patient
  • Assists patients in working with governmental agencies to apply for and receive benefits, such as Medicaid
  • Assists patients adjust to changes and challenges in their lives, such as illness, aging, loss of independence
  • Acts as a liaison with outside agencies
    • Consults with internal and external physicians, health care providers, discharge planners, and outside agencies regarding continued social service needs
  • Promotes patient engagement and shared decision-making
    • Interacts with patients and family regarding individualized patient/family post hospital and social needs, including advance directives
    • Communicates to patients in a manner that is understandable

 

 

Additional Responsibilities:

  • Performs other incidental and related duties as required and assigned.

 

Physical and Mental Demands:

  • Routine desk and office activities
  • Ability to move frequently throughout the facilities and to sit for long periods of time
  • High level verbal and written communication skills
  • Above average ability to manage multiple tasks  and projects simultaneously

 

Occupational Exposure:

  • Low risk exposure to bloodborne pathogens and chemical hazards.

 

Job Qualifications:

  • BSW required
  • One year of social work experience, including discharge planning required
  • Knowledge of federal programs and community resources
  • Excellent communication and problem solving skills
  • Strong customer service orientation
  • Managed Care experience preferred
  • Medical social work experience preferred
